Everything you need to know about converting and reading serum phosphate lab values.

Multiply the mmol/L value by 3.0974, which comes from the atomic weight of phosphorus (30.974 g/mol) divided by 10. For example, 1.10 mmol/L × 3.0974 = 3.41 mg/dL. To go the other way, multiply mg/dL by 0.3228.

A commonly cited normal serum phosphate reference range for adults is roughly 0.81 to 1.45 mmol/L, equivalent to about 2.5 to 4.5 mg/dL. Ranges vary by laboratory and age group, children and adolescents typically run higher, so the range printed on your own lab report is the one that applies to your result.

Countries using the International System of Units, including the UK, most of Europe, Canada, and Australia, report phosphate in mmol/L. The United States conventionally reports it in mg/dL. Both describe the same blood concentration, just on different numerical scales.

A result below the reference range is called hypophosphatemia and a result above it is called hyperphosphatemia. Both can affect bone, muscle, and kidney function.
